It’s not YET in Diablo 4. The game’s development chief Rod Fergusson hinted it may become available in the future! Either through a future base game update or as Season Pass DLC.
To quote him: “We wanted to make sure it felt authentic to the kind of gothic, dark themes we have. We’ve been really focused on trying to keep it as grounded as possible. And because of that, there’s no secret level in Diablo 4 that people might be looking for as per previous games. But that doesn’t mean there won’t be in the future.”

So if it’s still hidden under lock and key, where will we be able to find the Diablo 4 Secret Cow Level?
Datamines haven’t really uncovered anything substantial. But that may be because the developers haven’t turned on the necessary items to open the Secret Cow Level with in the game. So it may already be coded in, but Blizzard just needs to flip a switch to make the needed items droppable in-game. We may find out later on in Season 1 2 or later.
That won’t stop Diablo IV fans from speculating endlessly, though, the Secret Cow Level must exist!
