Does the game save each Left Arm with ID that you cut off and collect?

Not right away. But yes, the game will save the Left Arms you just collected at the end of the chapter you’re in. That means you must finish the level to save your progress. If you die or restart the level/mission/chapter, then you might have to collect some items all over again.

How do I get the reward for the Left Arms I’ve collected?

The trophy/achievement will unlock once you complete a chapter where you found the last Left Arm ID (most likely the last chapter). They will pop up during the Results screen, and not when you pick or collect the last collectible.
