Finding all 16 QR Code locations and scanning them will help unlock the following Achievement / Trophy:
* “Read-only” (15 Gamerscore / Bronze Trophy) — Complete the final mission in the QR Code Investigation.
* Bonus: After finding all QR Codes you’ll unlock the QR Code Investigation Mission. You’ll need to finish it to get the above mentioned Achievement / Trophy. Your second reward will be a new Tommy Gun weapon!

How to unlock QR Codes?: The QR Codes are plastered throughout the city landscape. Find a camera to align all sections of a QR Code. Once aligned, hold the on-screen action button to scan the code and download a hidden audio log. Find all 16 QR Codes to unlock the related special mission.

How to find missing QR Codes?: To reveal the code locations on your map you must unlock all ctOS towers. The displayed locations aren’t very accurate though and you must find one specific surveillance camera and look at the QR Code from a certain angle. The correct camera is sometimes far away, or can only be activated from an elevated position.

Tip: None of the QR Codes are missable and you can still get them in free roam after beating the story campaign.

Does the game save each QR Code you collect?: Yes, the game will automatically save the QR Codes you just collected if you scanned the QR Code in question, so you don’t have to play to the end of the mission if you’re in one. After obtaining each collectible you can safely quit the mission without losing collectibles progress.
