Important Tip: The “Insignia” trophy / achievement will only unlock after you complete the “Ground Zeroes” story mission. Shortly after the credits are over, you will see a screen with your rewards. That’s when it should unlock.

Unlockable Rewards: Collecting the XOF Patches will also give you a nice reward. If you’re playing on the PS3 or PS4, you unlock the “Deja Vu” Extra Op mission (starring Solid Snake). And if you’re playing on the Xbox 360 or Xbox One, you unlock the “Jamais Vu” Extra Op mission (starring Raiden).

How to find missing XOF Patches?: All of the XOF Patches are scattered across the “Ground Zeroes” mission. That’s the only main mission in the game and it’s available from the beginning on.

Does the game save each XOF Patches you collect?: No, your collectibles progress isn’t automatically saved. After picking up a collectible you need to reach the next checkpoint to save the progress. If you die before saving the game, you will have to collect it again.

Note: The last XOF Unit Patch (#9 in the video guide above) requires you to roll around on the ground, and although it’s the last here, you can actually get that at any time you want by following these instructions… Hold the X button on PlayStation consoles or the A button on Xbox consoles, then aim your weapon and hold down the left stick to start rolling. This will initiate a cutscene and the last patch will fall from the sky. Be sure you are somewhere outside when doing this. The other 8 unit patches can be picked up like normal collectibles, but they are hard to see (Protip: Use your Nightvision goggles to make them glow in the dark). Pause the video and put a marker on your map to locate them easily.
