In the Metal Gear Solid 5: Ground Zeroes trophy guide we’ll show there are 15 Trophies (0 Hidden) that can be earned in the PS3 and PS4 versions. Earn Bronze (13), Silver (1), and Gold (1) Trophies to increase your Gamer Level.
Note: The game doesn’t have a Platinum Trophy due to Sony’s regulation of not allowing it in non-full price games like this.

1. Reunion (Bronze) — Reunite with Chico or Paz

3. Genesis (Bronze) — Clear a Side Op or Extra Op
4. Accomplished (Bronze) — Clear all missions (including Side Ops and Extra Ops)
5. Skilled (Silver) — Clear any mission (including Side Ops and Extra Ops) with a S-rank
6. Hero (Gold) — Clear all missions (including Side Ops and Extra Ops) with a S-rank

7. Rescue (Bronze) — In the “Ground Zeroes” mission, rescue the prisoner to be executed and extract him via chopper

Location: The prisoner does not appear until after your mission has progressed past saving Chico. — Down the hill from where you start the mission, make your way to the far-left side beyond all the tents. Be on the lookout for a pile of crates, next to them the prisoner in a yellow jumpsuit will be lying down.

Pick up the prisoner and carry him back to where you started the “Ground Zeroes” mission. By the way, it’s a good idea to call in the chopper via the (in-game) iDroid menu before you reach the starting area (for example: once you get out of the compound gates again), so that the chopper will arrive shortly after you get there.

8. Depth (Bronze) — Clear the “Eliminate the Renegade Threat” Side Op by extracting both targets

Requirement: You will need the M2000 SNIPER-D given to you at the start of the mission.

9. Pacifist (Bronze) — Clear the “Intel Operative Rescue” Side Op without killing a single enemy

Note: The Intel Team agent VIP is actually the Metal Gear series creator Hideo Kojima!

Load up the mission “Intel Operative Rescue” on Normal difficulty.

To unlock it, you will need to make sure to not get any kills during the level. Use you non-lethal pistol to kill ground units, immobilize any vehicles, and take out the gunner in the side of the chopper throughout the entire mission.

The only time it is ok to use anything other than you pistol is when the armoured vehicle comes out, which is when you will need to use the RPG that you found near the Operative. It is important to note that splash damage from the explosion may kill neutralized enemies near the armoured vehicle, so make sure you take it out when it is not near any of the guards on the ground!

10. Infiltration (Bronze) — Clear the “Classified Intel Acquisition” Side Op while riding in the back of a truck

11. Extraction (Bronze) — In the “Destroy the Anti-Air Emplacements” Side Op, rescue and extract all prisoners via chopper

To unlock it, begin the mission “Eliminate the Anti-Air Emplacements” on Normal difficulty.

There are a total of 4 prisoners that need to be rescued in order to unlock the achievement/trophy. The first 3 are found in the camp near the spawn. Just neutralize the enemies, and carry them down to the chopper landing spot.

The final prisoner is a little more difficult to attain, but can be found inside the holding cell where Paz was found in the main mission of the game. Plant C4 on the Anti-Air establishments, and then grab the prisoner and make your way to the exit while blowing up the Anti-Air units as well as the armoured vehicle that shows up afterwards.

The chopper will land at the extraction zone, and you’ll need to load all 4 prisoners (alive) into the chopper, to gain this award.

12. Reminiscence (Bronze) — Recreate all scenes in the “Déjà-Vu” Extra Op

This is a PlayStation exclusive mission that you unlock after you collect all 9 XOF patches and complete the game’s “Ground Zeroes” campaign.

Your task is to interact with 7 objects in order to see flashbacks of the original Metal Gear Solid PS1 game. These objects are pretty close together, so it shouldn’t take long. Avoid being detected in the beginning and go to the helipad in the northern part of the map. If you kill the guards in front of the helipad or destroy the helicopter you won’t be able to trigger the flashback anymore.
Some of the other scenes can also become missable if you destroy their respective objects (jeep, tank, camera).

When pressing the options/start button you can scroll over to the mission objectives to see which scenes you have completed. You need to do all of them in one playthrough. The trophy will pop after finishing the mission. To end the mission you need to crawl through an air vent near the gate in the north.

13. Unlocked (Bronze) — Unlock all trials

Tip: How to unlock all trials in Metal Gear Solid 5? You have to play each mission in Hard difficulty mode around 3-4 times in order to unlock the hard trials.
Then for the normal missions, since all the first columns says is “mark all enemies”, you just have to do exactly that! Mark all enemies in all the missions, then at the end of each mission it should say “new record” and the time it took you. Don’t worry if it takes to long, because at the end you’ll unlock the 2nd mysterious trial which is called “FASTEST NEUTR. OF ALL ENEMIES.” So do that for all 6 missions and you’ll reach to 100% completion!
