You must complete all missions (including collecting all diary) and defeat all wooden dolls! after that you’ll find the 4th hokage on the roof of hokage’s top(hidden leaf)

Firs you must completeng all history mode, then you must defeat all characters including sasuke (classic and shippuden).
Finaly you must go to Konoha, to the Hokage street, go to the roof and defeat 4th Hokage

You should find the Yellow Flash on the TOP of the Hokage office.
Make sure you have completed all the Clone Fights! Also the four clones available after you beat the Sasuke RPG-mode
